Prisoner health records
Your prison health record is a paper or electronic record we create when you enter prison.
Each time you see your prisoner health team, they update your record.
Getting a copy of your prison health record
To access your prison health record, fill in the prison health record application form (PDF, 112KB) and either:
- email it to prisonhs@health.qld.gov.au
- post it to the Hospital and Health Service (HHS) who has your records.
If you're in prison, you can take the form and your ID to the prison health centre to get a copy of your record.
You can also use this form if you’re requesting access for someone else.
Leaving prison and your health record
When you leave prison, we’ll archive your paper record. The West Moreton HHS Central Archives Team stores prison health paper records.

Health records outside of prison
Your prison health record may not include details about all public health care you have while in prison. For example, if you want records from a visit to an emergency department, you’ll need to ask their Hospital and Health Service for your record.
